Why Post-Hospital Home Care Matters

The time immediately following a hospital discharge is critical. Without proper care and support, patients face increased risks of complications, medication errors, and hospital readmission. Our post-hospitalization services are designed to bridge the gap between hospital and recovery, helping your loved one heal faster, safer, and more comfortably.

Who Can Benefit from Post-Hospital Care?

Our Hospital to Home Care is ideal for:

  • Seniors recovering from surgery or medical treatment
  • Patients managing chronic illnesses or mobility limitations
  • Adults with cognitive impairments who need structured support
  • Families unable to provide full-time care during critical recovery periods
  • Including those returning home after cardiac events or heart procedures.

Post-Hospital & After-Surgery Care FAQs

Often the same day you're discharged. The earlier you reach out — even before your surgery date — the easier it is for us to have a caregiver ready to go.

Post-surgery care usually focuses on the specific recovery timeline and restrictions from one procedure, while post-hospital care covers recovery more broadly, whatever the reason for the hospital stay. Hillendale provides both.

Non-medical, in-home care like ours generally isn't covered by traditional medical insurance, including Medicare or private health plans. We do accept long-term care insurance — if you have a policy, we're happy to help you understand how your benefits can be applied.

We serve families across Alameda, Contra Costa, San Mateo, Santa Clara, Sonoma, Marin, and Monterey Counties. See our full list of service areas or contact us to confirm coverage in your area.
